Prior to seeing the doctor/PA you must have notified your insurance carrier that Dr. bishop is your PCP (Primary Care Physician). Any charges incurred prior to this notification are the patient’s responsibility

You must notify our office of any changes to your records, including but not limited to: your legal name, address, phone number, place of employment and insurance information.

We will be unable to see you on your initial visit if you do not have a copy of your current insurance card with you (if you are utilizing insurance), or on subsequent visits if your insurance has changed.

Insurance carriers we currently do not accept are:

Unicare

BCBS HMO

Tri-Care

Medicare/ Medicaid

North Texas Health

Workers’ Compensation

PHCS

 PacifiCare patients must be precertified with your insurance carrier PRIOR to receiving OMT (Osteopathic Manipulative Treatment).

We do not file secondary insurance- This is the patients responsibility.

Your co-pay is due at the time of your visit. If you are a private pay patient, payment in full is required at time of service by cash, check, or credit card.  If you do not have your co-pay or means to pay for the day’s services with you please call in advance to speak with our office manager to work out a solution.

 

If you feel that you may be late for your appointment with the doctor/PA please notify our office. If you are more than 10 minutes late for an appointment you may be asked to reschedule.  Please allow yourself enough time for your travel to the office.

 

There is a $25.00 charge on all returned checks.

 

Past due accounts will be turned over to a collection agency.  When your insurance has paid its portion or responded with an Explanation of Benefits (EOB) we will send you a statement for the balance which is due in 30 days.

If anyone on our staff treats you discourteously, please let our office manager know.  We, in return, encourage our patient to be courteous.  Disrespectful behavior will be grounds for dismissal from the practice.

Our office utilizes a Physician Assistant in order to provide our patients with an opportunity to obtain on a more timely basis.  You may be asked to see the PA instead of Dr. Bishop depending on the availability of appointment times.

 Medical Records may be sent free of charge to any physician or insurance co., However, if requested by patient to be received by patient there will be a copy fee. Please contact the office for these charges.
